Patients Doctor's office received a fax with personal information from Harbor Medical stating a provider Ordered a supply by prescription and requested Medical records of patient. No HIPPA form was attached nor were these supplies requested. Scammer has the NPI number and information for the Doctor also...upon attempting to call the number on the form (657-206-1019) there is a busy signal and the disconnect.
