My office received an "URGENT!!!" FAX requesting my signatures (as treating physician) and the last 3 office notes for one of our patients, in order to dispense medical equipment. The equipment being requested included Lumbar, shoulder and knee orthoses, NONE of which were ordered by me, nor requested by the patient. The request included the patient's name, DOB, home address, phone number, Medicare number, and height and weight.

The company was offering a back brace and knee brace at no charge. Validation of Medicare number and dates of Medicare A and B coverage. When they asked for validation of the persons name they provided my deceased mothers. They used 2 numbers to call from 620-464-5185 and (620) 873-6772.
