Type of a scam Healthcare/Medicaid/Medicare

Fax received in our primary care doctors office with a completed requisition that included all our our practice information and the patient personal demographic information. This included Medicare ID #, DOB, phone number and patient past medical history and a false exam note and letter of medical necessity that was prefilled out and ready to be signed. This requisition was for extensive genetic cancer marker testing to be sent to the patient and billed to Medicare. The cover letter stated that this test had been ordered by the patient and simply needed our ordering providers signature so that the kit could be mailed to the patient. Patient was notified and reported that he had never heard of this company and did not order any testing on his behalf. Phone number on the requisition never connects to an actual operator and has the recording that you are calling "Ring Central Conferencing".
