On 08/18/2021 I googled "badger care phone number" and called the first number that popped up (in an ad): (877) 986-1175. Before I got to operator, I had to press 1 or 2 for age range, then I had to press 1 or 2 for income range, then I had to enter my SSN. Then I got connected with operator (scammer) Jesse Nelson, and I was surprised that they were not Badgercare. But they could offer me and my daughter health insurance, where I could choose provider in any state. They took my full name and DOB, phone number, my daughter's full name and DOB, my address and zip code, my email address, offered me health insurance, vision and dental for both of us for $355/month + $125 sign up fee, took my debit card information and charged $480. I kept asking them, what is the name of the company or health insurance name, but they kept explaining that it is overall united group that works for all health insurance. Meanwhile and while they were "setting up my account and finding me in the system", by chatting they were asking me where I work, what my bank is, what is my pet's name, when did I became a citizen.

Description of this plan from their email:

"Association for Better Health (ABH) that includes the SecureCare Preferred Standard Limited Medical plan, underwritten through First Continental Life."

They sent me short text message on my phone from +1 (281) 214-6182 with link https://ensurancehealth.org/srt_url/r2528. They sent me couple of confirmation emails about member sign up and invoice about my purchase from [email protected] (these emails could be found only in spam)

Then they abruptly hang up and called me from different phone number: (888) 864-3611

They sent me another email from [email protected].

They asked me to click on the link https://ensurancehealth.org/srt_url/r2528, that they had sent me in a text text as well to my email (to enroll). They instructed me to click two boxes: with some file names on, but I could not see any files open to be able to read them; I was told I would be receiving plan description and everything in the mail (which I have not yet). They instructed me then to click "enroll" button, asked me to sign my signature using mouse pad. Then they sent me another text message from +1 (281) 214-6182 to my phone with code, that I had to "provide to agent".

After I was enrolled, they gave me Member Number and temporary password for me to sign in and change password.

Right after this conversation that lasted over 30 minutes (from two different phone numbers as mentioned above), I tried calling them back the number they called me from: (888) 864-3611, and I said I wanted to cancel everything I just signed up for. Different agent told me they would call me back, took my phone number, but never called me back.

Right after that I called my bank and cancelled my debit card, notified them about identity theft and personal info theft. They filed the charge on my account as fraud, but I have not received refund yet.

Next morning 08/19/2021 I filed police report with Madison Police Department.
