Received an EOB from my Medicare Supplemental isurance company USAA, showing they received multiple claims from Carebridge Innovations, whom, to our knowledge, we have never had any dealings with. Ten individual charges for $2,295.00 each for a total of $22,950.00. Five of these charges were duplicates on the same DOS. My husband's insurance company closed/denied the claims due to not receiving a valid W-9 form from the provider.
