Sent a bill in the mail impersonating UC Health in Cincinnati, stating I owed them a balance for an ER visit even though I have never even been to Ohio. Stated that the service location was “Ucp Jewish Hospital Kenwood”, and used the name of a real doctor practicing at UC Health. Used a fake impersonating website (my.uchealth.com, uchealth.com). Phone numbers used are 513-585-6200 and 1-800-277-0781. Email address is [email protected]. Full address for where they wanted to receive checks: UC Health, P.O. Box 630911, Cincinnati OH 45263-0911. They also had a section for income and residency information, and wanted it sent to: UC Health, Patient Financial Services - Correspondence Unit, 3200 Burnet Ave., Cincinnati, OH 45229-9983.
