Whether this is phishing for identity theft or insurance scam (or both) this arrived by postal mail. My guess is that they feel safer using post than online and that they may not have access to the best database. They are certainly very careful to avoid prosecution based just on this "invitation" but the intent to defraud in some way is clear. They want target's DOB, area code and phone number, SPOUSE'S Name and DOB, and if that weren't enough, please also provide your signature for us. I don't know what the next phase of this game would be but they don't directly ask for money yet, so it's likely about collecting the data. LMS never gives a full business name; they use a POB (very high number); they have no website and no way to directly contact them or to get more information. They also include (pt 9 font) a disclaimer about representing any government agency. Very slick. I don't know if anyone will fall for this (I did not), but I post here just in case.

I received a mailing from LMS asking me to fill out a card to apply for funeral expense benefits.

- Vancouver, WA, USA

called threatening "court documents being served and that I would essentially be sued for nonpayment of nonexistent debt" I called back and asked them to verify their information to see if they were legitimate, then they responded "see you in Court" then hung up
