Multiple faxes were received at our physician's office asking for a patient's medical information and a physician's signature, to approve medical equipment.

patients doctors office was contacted via fax that the patient ordered braces- bilateral wrist, bilateral knee, and back. There was no address or website on the paperwork. They did however have the patients information- name, address, phone number, medicare # and birhdate. I looked up the phone number and your scam tracker popped up. I called the phone number and it rang busy then disconnected. I then called the patient and she verified that she had not contacted anyone about any braces.
