I went on the website and I started out saying my weight and height, then it asked for my name so I put my name and then it started asking me for more information I wasn't comfortable with so I backed out. Then I got an alert saying I had a charge for 62.99 from Formulaappcom. I looked it up and it was the weight loss ad I had looked at from Facebook. About a year ago I had ordered some weight loss patches from a company off of Facebook and I think it's the same one. That's the only way I can see that they have my card number. I never did order or attempt to order anything from them. I called the bank to close my card number so it doesn't go through. I'm just glad my bank caught it. I'm not sure how they got my card number but it's a scam.
