I needed to fill out my US Passport application so I went to go find the application and the https://passport.uspassportandvisa.org/passport-v3 site actually looked legitimate. It allowed me to fill out the form, and told me that I can choose to pay the $130 now instead of at the appointment. Come to find out that it was a fraudulent site who now has all of my information since I submitted the completed form on it. The charge showed up on my credit card statement as EASY-FORM-FILLER.COM 234-5678900 MO for $125.83

My daughter was filling out my passport renewal form. Apparently she went into this web site to fill out renewal passport for ds-82. We went through the whole application and the charge was $132.07 paid with debit card. After we were done I noticed something suspicious about this website. After reading from the original state.travel.gov that they talk about these scammers. We did not submit the form.

A passport renewal application was filled out on Easy-form-filler.com and a “renewal fee” of $111.52 was charged. The charge was taken from the account, but the money was never received by the government passport agency.

I was in need of getting a passport. I used this website, Easy-Form-Filler.com, to pay for the passport form, filled out and ready to be processed. According to the site, all I had to do was print their form which was filled out and take to the post office and pay the additional $35 when at the post office. In fact, the hundred dollars I spent through this website was completely a scam therefore I lost $100.

Somehow I ended up on their site while I was attempting to renew my child passport online. I simply was on the government site and I was directed to their site thinking I was still on the government site. This site is acting as it’s part of the government site taking personal data. It charged me the cost of the passport. This was fraudulent and had nothing to with the official government.

I was helping my sister apply for a passport. Somehow this company charged my credit card without me knowing. I tried to call them to cancel; called three times, never got a person, left messages, never got a return phone call. There website said "email them" but there was no email to be found. These people should be put out of business.

I thought it was a legitimate site, stupid. I provided them with my social security number, father's name, mother's name, wife's current address, age, height, all the information requested on the DS-11 form for a passport, in addition to my credit card CV. Luckily, the bank did not charge the card. I don't know if they will use my identity to make loans in my name, that is, steal my identity for illegal purposes. The consequences of my lack of caution will be expected.

I thought it was renewing my passport, but I realized it was a scam after I paid. There was North Korea on the website which was not included in the .gov website. It told me to send a check to the US Treasury of State, which is not the correct department that should be sent.

The website offered US Passport renewal services. I filled out all the forms with sensitive information and when I went to pay, my bank sent me a text asking if I authorize the transaction. Foolishly, I said yes but then realized I was filling out this info on a .com site and not a .gov site. The payment page refreshed, I did NOT re-enter payment info and opened a new tab and Googled if that site was a scam and sure enough, it is. It's only been about 30 minutes since the incident but I am extra cautious and warning others before it happens to them.

This site charges money for you to complete passport renewal paperwork online, when there actually is no charge at the DOS government agency to print the paperwork for sending in. This site charged us $67.88 twice for 2 passport renewal paperwork applications, and I also gave them all of our personally identifiable information. I am horrified. The site looks very similar to the DOS government agency. We were not mailed ANY receipts of our transactions.

Once personal is submitted a pdf is emailed with your personal information and instructions on how to submit the form along with the actual passport fees to the government agency. This site appeared to be a valid passport application site. After completing the application the passport renewal form was downloaded and printed. I then received a credit card charge of $88.83 just for filling out the form. All personal information was given. I called the number provided on the forms and requested a credit for the $88.83. They said it would take place within 5 days. We shall see. I contacted my CC company and asked to dispute the charges, however, they said since I called the company and requested a refund I would have to wait 15 days to see if the scammers responded. My advice is not to call the company, contact your CC company and ask to deny the fraudulent charges.

EASY-FORM-FILLER.COM charged $59.70 for a free online passport application and made it seem like it was an official US Passport service. no one is answering the phone for service. I applied on 6/12/23 for my daughter with her SSN and other private info and feel this is fraud. why charge for free online application for US passport when it's free. the website is deceiving.

Website promises electronic submission of passport paperwork. It does not do that. It takes the information and prints it. Very much a scam.

This site has the appearance of being an official site. I was looking for the State Department Passport section and Google put this at the top of the search list. They ask for your social security number, which is not part of the form they print for you. It appears to be a phishing site for both a valid credit card and a social security number with associated information. They list a phone number but no one answers. They print out what appears to be a mailing label and they send follow up emails.
