Scammers used a CVS caremark letterhead for the cover sheet. They requested the Doctors signature and the patients chart notes for DME! Spoke with our patient and he said they were Medicare scammers. I typed in the phone number on the coversheet and seen this page.

Same experience, order faxed to us for all sorts of braces with all physician and patient information. Called requested medical, stated it would work as we had not seen patient in over 5 months. Faxed medical. Patient called stating someone from our physician's office called and said our Doctor wanted braces for him. No one from our office called him. Patient called a week later. Braces showed up, company called Rehab Supply phone 800-485-9717 x231 fax 407-214-4253. Total fraud.

sent a fax to our Medical office with all of our doctor's information and all of a patient's information requesting back brace, bilateral knee braces and suspension sleeves. They had all patient's insurance information. They requested doctor's signature and last chart note faxed to them at 407 214-4253
