Review of invoice found no website, phone number listed appeared to have been used by a medical service in TX, but when called says "the person cannot accept calls" then rings and rings, email appeared fake, and Business address was for a post office in TX.
Review of W-9 found that it was listed as an individual, but used an Employer ID which came up with no valid entries in CA or TX government websites.
