Company faxed a Wage Assignment Demand Notice to employer of victim indicating that employee voluntarily signed up for a Wage Assignment when they applied for a loan. The story is that the employee defaulted on their loan and that Apex is attemtping to recoup that money by collecting 15% of the employee's check. Employer is supposed to send a paper check to the address: *** d/b/a Inbox Loan, PO Box 637, Blanding, UT 84511. Company doesn't have a website and cannot do money transfers, accept credit cards, or accept any kind of electronic payment. Apex was even able to provide the loan agreement that the employee supposedly completed which allows Apex to collect this money. The loan agreement shows that it was electronically signed by employee at an IP address that matches her place of work, which is the same place the fax was sent. The company has a lot of the employee's personal information including address and SSN.
