Erica with Better Health Medical trying to get medical record chart note on a patient in order to provide the patient with a NEW glucose monitor. I contacted the patient and they knew nothing about it and they have a glucose monitor that is in good working condition. Patient stated they do not need a new glucose monitor at this time.
