This is another Medicare scam - trying to charge Medicare for unnecessary testing and supplies. It has patient and provider information and is stating patient is requesting these orders, when neither they nor the doctor had requested them. The tests/supplies are wildly unnecessary, and would get billed to Medicare, and likely never delivered at all. These types of scammers simply hope doctors will sign off things without looking closely enough at them. It's pure evil, and extremely common.
