On 2/23 around 10:14 AM received a phone call from 901-310-1303 on house phone. Call was from "Medi Assist". Caller claimed there were

financial assistance programs through Baptist Hospital East. The caller gave a reference number and a callback number.

Reference number given:103455345

Call back number: 866-758-1358

Caller used my first and last name.
