Almaz Med Supply Inc. attempted to charge my Medicare $2,606. for medical equipment that was never ordered. This is the second such scam I’ve experienced on my summary notice. Fortunately, Medicare must be on to the Almaz scammers because they didn’t pay. The other scammer, Vivital Inc. got well over $2,000 cash from my Medicare account.

We have no contact with this company. Do not do business, or receive any services from Almaz. This fraud was reported to Medicare on March 17, 2025

I am a victim of Medicare fraud by Almaz Medical Supply in the amount of $8,280, of which Medicare paid out $6,333.48 for gel wound pads. In speaking with Medicare on 3/5/25 and again on 3/18/25, I was told that the claim was submitted with all necessary medical information and backup covering three claim dates: August 15, 2924; September 16, 2924 and October 16, 2024. Each claim was for $2,760. I never spoke on the phone to anyone, nor did I respond to any emails regarding medical equipment.

A company called Almaz Med Supply has submitted a claim to my Medicare account, for glucose monitors. This is NOT something I use, nor did I receive anything. Medicare paid $2,005.74 for this claim! I have called Medicare to report the fraud. The claim includes the doctor who supposedly ordered these durable medical supplies, who they say is Tony Kannarkat, and he is a real doctor located in my city. I plan to call his office, too, because he may not know that they are using his name as the ordering physician. I looked up the Almaz Med Supply business on Google, and it says "Permanently closed"! Since someone else reported this company name, it appears they are committing fraud with other people's Medicare numbers as well. I was told by Medicare that after the investigation closes, Medicare may send me a new card with a new number. I found out recently that my data was included in a huge healthcare business data breach, so my guess is that's where this fraud originated.

A Summery notice from Medicare listed charges by Almaz Med. The charges were suspicious, and I phoned Medicare and made a fraud report. I never had interaction with the company. Three charges listed were identical in description and cost for wound dressings.

I received my Explanation of Benefits statement from my supplemental Medicare insurance company. I saw 9 dates of service listed on a monthly basis from May 2024 through December 2024. These apparently were for some sort of rented medical equipment. I called my supplemental insurance company to informat them that these were wrong. I had already looked up the "service provider" who still exists on the Internet but as "permanently closed." On another site "BBB scam" I saw that another person had already reported in February a similar scam. My supplemental insurance company is now aware of this and they have already funded their share (the coinsurance amount) which I guess for them is a loss. She told me to contact Medicare to inform them about their share of the payment loss as well, which I will do during business hours. By the way, when I accessed their "website" which appears to look like a real company in the USA except for in the corner a popup came up with Russian letters on it.

This company billed Medicare for medical supply items that were never ordered or shipped. The product mentioned was glucose monitors which I have never used since I have no need for this product. I reported this fraud to Medicare.

Almaz Med Supply submitted fake claims for gel wound pads to Medicare and AARP United Healthcare for dates of service 8-13-2024, 9-12-2024 and 10-15-2024. This is blatant Medicare fraud. Medicare says the claims are under review and they haven’t paid anything to Almaz yet.

Received a Medicare statement showing charges for Almaz Med Supply from NY. Medicare paid the claim in my name which was a bogus charge for me. I live in PA and never was in NY.

This was on my medicare statement. Almaz Med Supply, which is located in NY, billed medicare for diabetes services. I do not have diabetes nor did I receive any of the services medicare was billed for. The services billed were from May - Dec 2024. So far medicare has not paid anything to them. The claim is under review.

I received a Medicare Summery Notice in the mail on 2/16/2025. There were billing dates of 8/20/24, 9/19/24, 10/21/24 for 120 Alginate or other fiber gelling dressing, wound cover, sterile pad size more than 16 sq inch. $2760.00 was billed to my Medicare for each of those three dates. Medicare paid $6333.48 to Almaz Med Supply, Inc. I did not use or receive these items. My claim said these were ordered by Lia A Fiallos who I do not know. I tried to call the phone number listed and my call was never returned after 3 messages were left. Lia A Fiallos was not found in their employee directory.
