This was on my medicare statement. Almaz Med Supply, which is located in NY, billed medicare for diabetes services. I do not have diabetes nor did I receive any of the services medicare was billed for. The services billed were from May - Dec 2024. So far medicare has not paid anything to them. The claim is under review.

I received a Medicare Summery Notice in the mail on 2/16/2025. There were billing dates of 8/20/24, 9/19/24, 10/21/24 for 120 Alginate or other fiber gelling dressing, wound cover, sterile pad size more than 16 sq inch. $2760.00 was billed to my Medicare for each of those three dates. Medicare paid $6333.48 to Almaz Med Supply, Inc. I did not use or receive these items. My claim said these were ordered by Lia A Fiallos who I do not know. I tried to call the phone number listed and my call was never returned after 3 messages were left. Lia A Fiallos was not found in their employee directory.

Wife was contacted via telephone and was told that she was receiving a back support item. that a doctor told the company she needed one. A back support idea was shipped to wife's address, and then wife's Medicare was charged $1290.70, of which Medicare approved $530.06 and paid $265.89, and the Medicare statement stated wife may be liable for $258.74. No doctor's name was provided with the back brace package, however, on Medicare statement, a name of Brendan C Albrecht was provided. We do not utilize services of anyone with that name.

Had a $6,600 charge show up on my health insurance from Gazar Recovery INC, noticed it by my EOB. Said it was for KO Double Upright Prefab OTS. Looked it up online and it is some type of knee brace. I don't have issues with my knees and i have never heard of or been to Gazar Recovery. Luckily my health insurance company denied the claim and i will be calling them to make sure nothing is paid to Gazar Recovery.

It looks as if my insurance information has been breached. I received an EOB from my insurance company stating that I received services from GAZAR RECOVERY INC. on 11/06/24. I live in another state so I never was at this location and never received any Orthotics. Total Charge was $9,500 and the insurance paid $2,565.44. Have not been billed from Gazar Recovery as of yet. I will be filing a fraud claim with the insurance.

I received my Medicare EOB on December 9, 2024. I immediately saw a fraudulent charge from Absolute Medical Supplies for Ostomy pouches ordered from a doctor i have never seen. Of course, I have no medical condition that warrants the use of Ostomy pouches. These charges were submitted to Medicare between February 9- June 10, 2024. This is clearly a Medicare Scam. Medicare paid the claim. I immediately alerted Medcare. They removed the charges from my account and said they would investigate. I then called the doctor's office, a very prestigious university hospital. They were not aware of this scam, but did say they would investigate and get in touch with Medicare. I can only hope this leads to the closing of this business and prosecution. They are stealing from the federal government.

This is through my Medicare account. They have billed my Medicare account for monthly Urinary catheter services that I have not received or need. Medicare has paid, per my EOB, $8034 for services May - Oct 2023. I have notified the Medicare fraud department about this, and they have issued me a new Medicare account. My Medicare was also billed by other companies for Covid test kits that I didn't order or receive in 2023. I also reported those cases as well to Medicare Fraud department. I saw this on ScamPulse.com and wanted to let you know as well. I am just one individual, so I am guessing Medicare is being defrauded for lots of money, which also affects all of us.

Fraudulent claim on Medicare for 800 urinary catheters which were never ordered. Reported to Medicare as fraud and abuse in December 2023. Medicare was charged $7760 and paid $5832.96. There is an ongoing scam with G&I Ortho supply in Brooklyn as well. Reported on CBS news. Please check Medicare summary notices since these supply companies are using stolen Medicare numbers.
