This is Medicare fraud. I opened a recent Meidcare claim statement today to see services I did not use, from a provider I have never seen, and for medical supplies for wound care and glucose monitoring device, which I do not require. The Medicare number, my name and address were all on the claim statement for these services which is extremely worrisome. I have reported the fraud to Medicare for an investigation, but this company is a known perpetrator of Medicare fraud. I alert everyone to read their Medicare claim statements to verify that you received the services listed.

Almaz Med Supply Inc billed my Medicare insurance company and my military insurance for over $30,000 for services / supplies that did not apply to me. I contacted Medicare and reported the incident to the fraud claim office. Fortunately for my military insurance they did not pay anything that was billed to them. The people in this company needs to be arrested and sent to prison.

Noticed Alamaz had billed medicare, via my medicare account, for two services I never ordered or received. The total was over $10,000 in 2024. They never attempted to bill me for "my responsibility". Obviously it was a medicare scam. My secondary insurance was suspicious and did not pay them as they failed to respond to their requests for more information.

This company, Almaz Med Supply, did not contact me. I found fraudulent charges/scam on my Explanation of Benefits from my Medicare Supplemental. Almaz made 3 claims billing Medicare $8,280.00 for wound dressings. My Medicare Supplement Mutual of Omaha did not pay anything. I did not receive any products. I do not have a wound. This was not something I needed or wanted. Almaz billed Medicare for unnecessary and unrequired products that were never shipped.

When I got my statement from my medical insurance provider there was the charge for $8280.00 of which Medicare paid $6333.48 and the provider was listed as Almaz Medical and surgical Inc.This was billed over a 3 month period from 8/6/ 2024 ,9/5/2024 and 10/07/2024. When the balance was sent to my medical insurance they didn’t pay anything due to “ requested information was not received” from them. How do I notify Medicare of this scam? I feel my Medicare information was obtained in order for them to make the payment to this scammer.

They stole my Medicare information and charged Medicare $26,846.00. Medicare approved $25,504.79, and paid $19,995.71, leaving $5,100.96 to be paid by my supplemental insurance company, or by me. My supplemental saw that the company was flagged when they went to process the claim and refused to pay it. I Googled the name of the company and found that there were a lot of reviews stating the same thing that I have written above, in varying amounts, but all in the thousands of dollars over a period of months. They also used doctors names to complete the order process. I have never seen or been to Dr. Raja Ram as a doctor. I called his office and they had spoken with someone last week, who had called about the same thing. Their office nor the doctor knew that his name was being used to order product for people who he had never seen, didn't have a need for it, and didn't receive any of the items ordered. I prefer not to talk to the media at this time. I would like to see someone in a high ranking position to take action before these scammers can get away.

Almaz Med Supply Inc showed on my secondary insurance EOB for $8280.00 billing for medical supplies. This company is unknown to me and I did not request or receive any services, supplies or equipment from this company. My secondary insurance asked Almaz for additional information and did not receive a reply so they did not pay the claim. Medicare processed this claim in February 2025, paying $6,333.48. I reported this claim as fraud to both Medicare and my secondary insurance.

on 2 separate occasions almaz billed medicare $24 thousand per occurrence for durable medical equipment that was never received and would have been medically unnecessary. I didn't find out about it until I got a benefits statement from my medicare supplement insurer. The referring physicians involved were unknown to me, never talked to me, and never provided any services.

This is the second time that this SCAM has come to my attention. It may have occurred even more in the past but did not come to my attention until the previous Claim Detail. This company bills insurance companies for supplies never rendered.

I previously advised by primary physician's office, Premier Medicine, Middlebelt Rd., Livonia, MI of this fraudulent company. They had never heard of them so the fraud must be committed directly with the insurance company.

Please stop this Company!
