This company billed Medicare under my account for $18,180.00 and Medicare paid out $11,448.00 in equipment charged each month from May 2024 through October 2024 at $3,030 each month. NOTIFIED MEDICARE when I received their statement for period March 5, 2025 thru July 5, 2025. I notified fraud on July 11, 2025. I had previously notified MEDICARE about this same company and fraud back in February 2025 for SAME COMPANY ALMAZ MEDICAL SUPPLY INC. for equipment for glucose meter. Billed MEDICARE from May 2024 through November 2024 for $251.00 each month for total of $2,606. MEDICARE PAID THAT BILL I ALSO contacted MEDICARE about this fraud back on February 17, 2025 when I received the statement from MEDICARE.

Company fraudulently billed Medicare Part B (and me) for services I never requested nor had. Specifically for urinary catheters beginning of June 10, 2024 and thereafter until October 9, 2024. This fraud was reported to CMS on July 11, 2025. They will investigate this further. Suggest you monitor your Part B Medicare Summary for more instances of this fraud.

Almaz medical supply in Woodside NY charged eight months’ worth of catheters to my Medicare account, which I saw on my Medicare summary received yesterday. Each month, March through October last year, they charged 300 catheters with insertion supplies to the tune of $3030 each month for a total of $24,240. It appears Medicare has already paid on these charges, but I don’t know anything about Almaz, I’ve never used a catheter in my life, and the doctor that they say ordered them on my behalf was a Dr. Shane Breeze-Baldauf in Milwaukee,Wi. I have never heard his name before. I have called Medicare about it as well as Baldauf’s office.

My Medicare account was billed for fraudulent claims for various medical supplies/devices by Almaz Med Supplies. The first EOB was in December 2024 with charges for a diabetic monitor and monthly diabetic supplies for the period May 2024 to Nov 2024. Total charges amounted to over $2,600 or which Medicare approved and paid over $2,000. I am not diabetic and do not have a diabetic monitor.

Then in March 2025 there was a new Medicare EOB for gel wound covers for the period Aug 2024 to Oct 2024. Total charges amounted to over $8,200 of which Medicare approved and paid over $6,300. I did not have any wounds that required these items nor did I receive anything at my residence.
