The scammer sent out a form called the National Social Security Survey asking the recipient to fill out the form and then send a $16 donation to the company. They will send this information multiple times asking for money each time. I also noticed after the incident that the address for the company was not the same on the second survey as it was on the first. After further research I found out the company was not legitimate and that it was actually a scam
