This was an attempt by BAKARIKA CORP to defraud a Medicare Advantage company. The dates used were Dec 19, 2025, and Dec 9, 2025. The Medicare Advantage company caught it and flagged it as fraudulent.

I received an EOB in the mail from my health insurance company where a claim was filed in my name using my address, phone, and insurance member ID to pay out Bakarika Corp for durable medical supplies from Jan 6, 26- Feb 9, 26. I have not ever had contact with, business with, consented to anything for any medical supplies from this or any other business. Bakarika committed medical fraud using my personal information and Bakarika corp is has also committed identity theft in doing this. And they have done both of these things across state lines which makes it a federal crime. I have reported this to Identity theft.gov, to the credit agencies and to my insurance company. I faced complications with getting a new medical ID and having my information transferred over from my previous ID to the new ID costing me hours of my personal time.
