I received an enrollment form for credit monitoring. It says there was a data security incident @ IAMS which may have impacted my personal information. It tells me to place fraud alerts with a Consumers Reporting Agency & names nine of them. It gives me Dec 24, 2024 as the deadline to enroll. They have sent these letters before. I became suspicious, because they also sent one to my husband who has been deceased for 2 1/2 yrs.

Received a letter from Southcoast Health saying my info including my social security number (SSN) had been compromised and offered a year of free credit monitoring with Identity Defense and they are asking for my personal info including SSN. I looked up their contact info and googled their number (1-866-622-9303) and a result came up with ScamPulse.com as fraudulent.

Received letter in mail, April 29, 2024!

I have no connection with this bank at all!

I received a Letter from Centennial Bank saying there was a notice of a data breach, letter dated April 19, 2024. In letter it says to enroll in credit and identity monitoring, gives activation code, enrollment date, and service term!

I went online typed in Centennial Bank and saw that there was a scam going on and that’s why I am filling this form out.
