I recently received an Explanation of Benefits claim submitted to my federal employee health insurance plan by a provider with whom I am unfamiliar. I have never received services from this provider, nor have I authorized anyone to obtain services on my behalf.

I believe this claim may be fraudulent and would like it investigated as soon as possible. Please provide details of the claim, including the provider's name, date of service, amount billed, and any supporting documentation submitted.

I would also appreciate confirmation that this matter is being reviewed and guidance on any additional steps I should take to protect my account and benefits.

This corporation has now filed TWO fraudulent health insurance claims using my daughter's PHI. I have no idea how they were able to obtain her PHI but they have it and they are using it. In the first case my health insurance company denied the claim. Currently, the claim is being reviewed and I have contacted the insurance company to let them know this is a fraudulent claim. I have also reported them to both the Texas and Maryland departments of insurance. This is criminal behavior and they must be stopped.

This is the 2nd time this year this same company has tried to bill my Medicare Advantage plan for urinary catheter supplies and now also wound care supplies for $5031. In January it was for over $12,000. Please catch these fraudulent thieves and throw them in jail! I had another fraudulent company also this year as well for over $15,000 and they were from TX. and were arrested trying to get on a plane in LA headed back to Russia. They were charged for millions and millions of dollars in fraudulent business claims from stealing from Medicare Advantage plans. Please seniors be aware of your medical bills and charges!

They used my medical insurance and billed me / Medicare Advantage plan for three different incidences of supposed medical supplies.

Luckily my insurance company refused to pay them, but I'm very concerned about the fact that they have all my information. And I'm wondering what damage they could do in the process.

My secondary insurance is Medicaid and someone suddenly reported that I moved out of state and I'm presently fighting that to get my Medicaid reinstated. I don't know if this is related or just a coincidence.

But my identity is obviously been stolen and I'm starting to have all kinds of different problems.

So I guess beware that these companies can try to steal from your Medicare plan and be sure to read your claims.

I'm going to try to find out what I need to do to regain my identity and keep it private.
