Fake US Bank call claiming credit card fraud

My caller ID identified the call from a US Bank Branch. The woman's name was Jessica Adams, employee #BO2008, and stated someone had applied for a credit card in my name. She gave the last four numbers of my social security number. The number she called from was 417-588-4111 and Lebanon MO flashed on the screen. She stated the request for the credit card came from Ashville, NC. I did not give her any information but asked to speak with her supervisor as her dialect was difficult to understand. I was transferred to 'Addie', employee #31005 and she was not much easier to understand. I told her I would investigate further and get back to her. She gave me 800-905-8585 as the return call number. I was not able to get through because of a continued busy signal, not typical of a bank phone system.

I called my local US Bank and they confirmed it was a scam. I put an alert on Equifax for a year.
