Individual named Mike, reached out to my primary care practice, from "Walgreens Rep for Walgreens Pharmacy Medical Supplies stated they received patient signed medical form from PCP. Mike requesting patient last office note for blood glucose monitor and back brace through the Medicare benefits. Mike stated request is Time Sensitive. Please fax last office note to:

I spoke with patient, who denies using Walgreens and does not need the monitor or back brace. We (the primary care practice) have never sent visit notes to a pharmacy and certainly the urgent request was suspicious.

The phone number left with us was from TOP DME SUP LLC in Albany, NP from Google. Unclear this exists.
