I received a fake invoice from Teton Radiology diagnostics in Salt Lake City, UT. (I live in Idaho) It was a bill for $459.00 for "Office Outpatient New 45 Minutes". It was mailed to me at my parents house, not my primary residence. It had a pay online option www.epayitonline.com as well as a catpcha code to scan for a mobile payment. It also asked for updating of personal information for patient files if mailing a check in. I was not able to locate the business online and the 888 phone number comes back to several different bill pay organizations on a Google search. I did not call the 888 number nor did I scan the captcha code.
