The scammer falsely claimed to be associated with Eden Emergency Medical Group, Inc. and included two P.O. Box addresses in the document sent to my residential address. The first was listed as: P.O. Box 869, San Dimas, CA 91773, and the second as: P.O. Box 744831, Los Angeles, CA 90074.

They initially contacted me via text message, followed by mailing a set of documents to my home. I reached out to both the medical provider and the hospital billing department, and neither found any outstanding balance on my account. Additionally, neither party recognized the information included in the documents I received.

I received a bill for emergency room services level 4 on 7-23-2004 for gout. The provider was Omar Ferrari MD at Providence Santa Rosa Memorial Hospital. I don't have gout and was not in the hospital that day. There is no mention of my Medicare and medi-cal insurance. I called the providence billing dept. and they have no record of the visit. They misspelled my last name on the bill, however my address is correct and providence told me the billing address in Pasadena is their correct address, they have a tax id that doesn't match mine on my last 1099 form, so I don't think they have my SSI#. It's strange that the billing address is correct, I wonder if an employee is involve with this scam! The letter also threatens collections if it's not paid by Sept. 1st. I took a picture and can attach it if you want to see it.
