I work at a merchant services company, and received a call from one of our merchants who asked if a form that she received was legit. The form was from Merchant Services Washington and requested pertinent info such as legal name, address, EIN, phone number, identification, voided check, bank account info, etc. The form stated "Final terminal security upgrade request form to be completed and faxed in before the Merchant Services Washington Deadline July 26, 2019." The form contained all of the information necessary to fill out a merchant processing application. The form is attached and I have removed all sensitive information.

I advised my customer not to send in her information to Merchant Services Washington as it was likely that they were phishing to flip her business to them or to potentially steal their money.
