Instead, this entity, posturing itself as an official, state-affiliated agency (which it is not) is attempting a money grab in the amount of duping unsuspecting business owners out of $150 bucks.

Hello,

Your online features to file a complaint appear to be nonfunctional. There are already about 56 complaints coming from throughout the US for a company that targets LLC holders—calling itself CPS in my letter, but apparently assuming other names. They have already been convicted in Colorado, from what I see on the ScamPulse.com website.

I have their deceptive notice in front of me (titled, "2025 - SAFETY MANUAL ORDER FORM") and would be happy to scan and send it.

It is purposely designed to look like a government form, with penalty warnings and a deadline.

Careful reading makes it clear they are not a government entity, but the intent is clearly to catch readers at the multiple, emphasized warnings.

We received a scam form in our regular USPS mail. It looks like an official Annual Report Instruction Form for 2025 for New Jersey LLCs and after speaking with my accountants, this form was meant to deceive me and I was told to shred and dispose of it. They want to charge us $195.00, plus $25 for any additional changes to file this form which we already do but the main concern and issue here is it looks as if it is a normal charge from the state of NJ.

This is clearly meant to imitate a state filing form and deceive its recipients.

The company, C.P.S sent me a "2024 Biennial Statement." The statement looks legitimate and appears to coming from a government agency. On the form, they have listed a customer ID number and my DOS ID number and the formation date of my business, which I understand is public knowledge. They claim that if I do not file my biennial statement I may be "prevented from certain business transactions." They do have a disclaimer that "C.P.S. is not a government agency and does not have a contract with any governmental agency to provide this service." Upon careful examination and googling the name of the group, I've learned that this is a potential scam. They want meto submit $150 to file the report. I know that it only costs $9 to file a biennial statement with the government. I don't want other businesses, especially new ones, to fall victim to this scam.

Informing our company that NY state requires a Biennial Statement once it becomes an LLC. They want us to send them a check for $150.00 to file, which, "includes state fee and C.P.S. processing fee."

It does state on the instruction form, which looks very official, that it is not a government agency, nor does it have a contract with any such agency.

I am very relieved that I read this paper several times, had a gut feeling this was not on the up & up!

I did not notice a website or email address. Just the mailing address and phone # to call for "further assistance"

They did not target an individual, but our business.

Received a "Biennial Statement" form for "9020 Avenue B LLC" using my PO box in Lytle Creek Calif. Payment information was for $125. I don't have a business of any kind, including in NY/NYC. Looks like this outfit is name is C.P.S. Please see my screenshot uploaded file for what they sent to me.

Received a 2023 - Biennial Statement Instruction Form that looks like a valid bill. Yes, it says in middle of page that it not a government agency and will file on your behalf, but it is designed to look like a legit legal document / bill, but it is for unsolicited and unnecessary services. These type of fraudsters have to be stopped.

Just like other reports, I recently registered LLC, and received a letter to pay $295.00 for "Federal OSHA compliance" write up. This letter looks like an official government invoice.

I went to https://business.dc.gov/lookup-business, and could not find "C.P.S." as a registered business in DC. Business has address at 5614 Conecticut Ave NW #190, Washington, DC 20015-2604 - and address for the UPS store!

The letter states [email protected] (a GMAIL address!).

I also found complaint on BBB: https://www.bbb.org/us/mi/lansing/profile/legal-document-help/workplace-complian...⇄ (read carefully numerous complaints).

Naturally, I did NOT pay this invoice, nor do I intend to.
BUT...

I reported this letter to my State Attorney General.

Scam for OSHA filing fee

I just received a 2022 Safety Plan Order Form stating I need to send them $295.00 to have them prepare and provide a comprehensive written safety plan.
This is bogus as it is just a plan not an official document.
Glad I didn’t fall for it.
It sure looks legit though!

Ive gotten 2. one in Sept and one in Oct. Can anyone tell me how long this scam has been going on? I have been at this job for a few months and have found past checks written to these people from the owners wife over the past couple years. 2022 Safety Plan Order

I received a letter in the mail from C.P.S. stating NYS require me to pay $295 for a third party to prepare a comprehensive OSHA policy. The email on file is [email protected]. What reputable business uses a gmail. I was scammed twice from this organization by paying my two years dues of $100.
