This scam is a postal mail form that is trying to look like an official form from the State of new Jersey, asking for Annual Filing and Reporting. It requires the user to complete the same steps they'd do with the State, but for $185 when the state charges $30. Presumably they file for you, but I doubt they address any problems or let you know if it doesn't go through. Dozens of people online report having fallen for this.
