This woman Anne came to our home with no appointment. She said she was following up with my son for Molina. She asked many personal questions regarding his care and doctor appointments and had my son sign and initial a medical ROI form. I called "Anne" after she left our home and informed her my son wanted to sign an opt out form. She said she has taken care of that and his medical records and benefits were not affected. I requested a copy of the completed ROI and the local address to her office. I have not received either. I called Molina; they said she is not affiliated with them. Her name being used is Anne Macharia.
