FRAUD

I have been receiving requests for medical Equipment such as wrist braces, back brace and a BP monitor for the same patient that our Dr. did not request nor did the Patient. They have been at it for the past 2 months. They 1st started with phone calls and since I told them I was going to report them for Fraud now they switched to faxes only. Be aware this is a Fraud, they have now gone as far a to forge our Dr. signature.

Received a fax to our company asking the medical provider to verify that the patient was indeed her patient and sign the form and date it.

Called patient and he said they had called him and wanted to provide him with knee braces, back brace and some other things and he told them he was not interested but they faxed us the request

He has medicare and is a veteran
