Letter, Direct Deposit information, W-4 form, and Proof of Address (Bank Statement, Internet, Phone, Water, or Electricity Bill) in PDF
format. Please also provide a means of identification (Identity Card, Driver's License, Front & Back copy) in JPG or PNG format, as well
as one picture photograph of yourself in JPG or PNG format. The means of identification should be on a white background paper and
all information should be clear and readable. The funds for the software will be provided for you by the company via check, the check
