Received a letter with FINAL NOTICE / COLLECTION AGENCY REFERRAL printed in red at the top. This is pretending to be a past due bill for a medical visit. The hospital network IS my network, but there's no date of service. I haven't had any services for $495 with my doctor. I checked the app associated with my doctor and see my balance is actually $0. The back is asking for a lot of personal information, including SSN and medical insurance information. I called the number and a recording said, " Welcome to Professional Billing". It was NOT my hospital network.
