is a scam address
the address belongs to a UPS Store. Frank Larose of the Ohio Attorney General is warning people of these scams and do not participate

Received this "Request Form" at our business address with the name of one of our customers on it. Looks like it may be an official request but when we googled scams this one came up and is clearly an issue. The address to send the money to is a UPS store located in Upper Arlington, Ohio. Can't someone go there and watch for these thieves to show up?
