Phishing email through employer insurance led to unauthorized charge

I received an email about a promotion that was through my employer-paid insurance agency. I verified through my insurance agency that the company that offered the promotion was legitimate. When I clicked the link, I read the terms and conditions (all of it, I always do). It noted that the program was completely free, but a credit card was required just in case I wanted to participate in additional programs. I was required to enter my personal information, health conditions, credit card, and my insurance information. I got a notice that I was approved for the program. INSTANTLY, my credit card was hit with $39.99 for SAMADIAPP. I did not authorize this charge. Thank GOD my bank declined the charges and notified me. I froze my card and now have to wonder who stole my identity. I notified HR and was told they didn't have to enter that information, so somewhere between the link, there was a data breach that directed me to a scam website.
