A letter was received addressed to my son from a company claiming to represent my health insurance. The letter requested information about medical treatment my son received and contained a date my son did seek medical treatment at a hospital. The letter said if I did not contact them within 20 days, my insurer "may be required to deny my claim". I contacted the company "Optum" to validate their association with my insurer, which they would not do, and claim numbers which they would not provide. They asked me for personal information about me and my son, which I did not give.

I then twice contacted my health insurance and talked to two representatives. They told me the claims had already been paid, that they had not heard of the requesting company, that I should not provide information to the requester, and that I should ignore the letter.
