I was first alerted of this by an outfit that my health insurance company hires to make sure claims that sound injury related can't be paid by using the responsible party. I had no injury and no medical bills at all that month so I went online to look at my explanation of benefits.

Sunny and Recovery claimed charges for a custom full back brace. A jointed hand brace, and two other items. Total fiction, including a diagnosis that had a small element of reality. $2520 + $3440 + $5270 + 2190
