The documentation looked and read very authentic. It was so close to the government forms that I did not catch it. After sending it in and them charging my credit card for the $150 fee, I received a binder in the mail. This is when I started getting suspicious as the state does not send things like this. Once I began looking into it I found out the address given is a UPS Store and the website does not exist. I did not try the phone or email.
