Website masquerades as providing online platform to request copies of your birth certificate from your state of birth. It collects all your data including name, address, city and state of birth, hospital of birth, mother's maiden name, father's name, etc. It even asks you how many copies you need and the purpose of obtaining those copies. It charges $49 making you believe copies will be on the way once you click submit. A few days later I receive an email with an attachment that has a one page fillable pdf file, which was the actual actual application belonging to my state of birth, with instructions on how to fill, print, notarize, send to your state with copies of notarized government ID and the actual payment and acceptable payment methods. $49 to paraphrase and repackage instructions available through few simple and intuitive online clicks at your state's health department website if you were to choose apply by mail option. Consumer BEWARE. They seem to have multiple online aliases/duplicate websites. Here's another: https://apply.quickvitalcertificates.com under the name "QuickVital Certificates".
