Email claimed to be State Farm following up on a recent claim. Offered $90 gift for taking a survey and allowed me to choose from a number of items as an incentive and only pay the shipping. Chose item and submitted Debit Card for shipping. Was almost immediately contacted by my Bank as a fraudulent transaction. They tried to charge my card $60. The Contact Us email is not real. In addition, it was channeled through 3 different 'companies'.

My bank had to issue a new Debit Card and open a case against the company for non-authorized transaction.
