RECEIVED "ORDER FORM" WHICH LOOKS LIKE AN INVOICE FOR 1,000 CT NITRILE MEDICA GLOVES

Received a letter in the mail postmarked on March 1st, 2025 with an origination location of Miami, FL 330. The return address was a faint stamp with "LEGACY MEDICAL SUPPLIES HEALTHCAREESSENTIALS" on the upper left of the envelope. The mailing name address on the envelope was correct for my business, but there was a phone number listed that was a previous number no longer in use.

Inside the envelope was a document titled "Order Form", with "ID number: 13-2406, Freight: FedEx, Date: 1/21/25" printed in the upper left. Below was a list of "quoted items" that included 2 cases of nitrile gloves at $689 each, and free FedEx shipping. The total was $1378, and a note was included below the total saying "Make all checks payable to Legacy Medical Supplies". There was a tearoff at the bottom of the document similar to what would be included with a check payment.

Of note, my business has never shopped with, ordered from, or contacted this vendor. Calling the phone number listed on the form led to an automated message directing me to email the email address listed on the form, with no option to speak to a real person. I did not attempt to email or otherwise contact this supposed business.

Received an Order Form in the mail that looks exactly like an invoice totaling $1,378.00 for Nitrile medical gloves, 2 units, 500ct each with unit price of $689.00. Item# LMS-53-245. There is a Disclosure statement that "...This is a solicitation for the order of goods or services...and not a bill, invoice, or statement of account due." However, the format of the Order Form is designed to make medical offices think it is a bill to be paid; in essence, purchasing 2 boxes of gloves for $1,378.00.

I tried finding a website for the company and got an error "store unavailable," so I knew something was off. Luckily, I saw another post on ScamPulse.com about this exact scam. Please don't fall for it!

My business address is listed as the shipping address for "Nitril Medica gloves" LMS-53-245 for $1,378.00

There is a disclaimer and a request to make all checks payable to Legacy Medical Supplies. We have not ever done business with this company and this "order form" invoice is not real

The company I work for received an invoice in the mail for 500 count medical gloves in the amount of $1,378.00. These gloves are way overpriced. We order medical supplies from two specific supply companies and have never used Legacy Medical Supplies. I tried to contact the company but I only receive an answering machine. This is a scam.

The letter listed a return policy and a request to make checks payable to Legacy Medical Supplies. It also included a slip to cut and include with the check.
