Our medical office has received multiple requests for medical records regarding medical equipment on a particular patient. The patient denies requesting the medical equipment. The requester also calls the office and insists on us sending the records even after we tell them the patient does not want the supplies. When we call the phone number back it is a disconnected number. We then received another request for medical records on the same patient from an unknown facility using the same fax number but no phone number.
