Consumer's parents received a letter from a place called Optimum Rx, but when they opened the envelope, it said it was from Publisher's Clearinghouse. Inside the envelope was a letter stating that consumer's parents were a second place winner in a contest, along with a printed check in the amount of $5970.15. The letter stated not to discuss with third parties. The phone number to contact the claim manager is 416-722-7958. The security code is PCH-04-WIN. At the bottom of the letter, there was a sample check that was labeled with Chase bank, but the printed check that was "to cover insurance and attorney fees", was from Wells Fargo.
