Provider at Dallas VA received a fax received a fax from One Source Pharmacy requesting he sign a prescription for a compound topical pain product for a patient whom he has never seen. Nurse for the provider called the company as they have sent several faxes although the provider spoke with someone previously and informed them that he did not know of the patient. In the fax was the provider's name, and NPI number. Nurse asked representative how did they get provider's information and she (Morgan) stated that they received the information from One Source and that the number provided on the fax is for a prescription service that works with One Source. Representative never would give me any other information such as the main number to One Source Pharmacy. Writer then called the patient who is listed in our database from 2012 and luckily had the same number. Writer asked the person (for the whom the prescription name was for) if he knew of the company and he said no. Writer then asked if he was currently seeing any provider and he stated no. Patient stated that he has not seen any provider since 2012. Writer then asked if he had seen anyone requesting pain cream. Patient then stated that he had gotten some email stating he could get free products such as pain cream, washing powder, back and knee brace and other products by filling out some health survey from once source. Patient stated that the survey also ask for his insurance information and he told them that he only had Medicaid which they accept. Writer informed patient to not give out such information on survey. But again the question is how did One Source get the provider's name, work address and NPI number when the patient did not provide such information and since the provider has never seen the patient.
