I received a letter from National Sweepstakes Company Inc. 9255 Sunset Blvd #420, Los Angeles, CA 90069 TEL 1-818-877-2158 informing me I had won $175,000.00 on the OHARA AMERICAN SWEEPSTAKES DRAW, File number UDI-04-856. I had never entered such a sweeps contest. (Also the letter was addressed to a fictitious name I sometimes use for mailing lists). A search of the file number leads to deception.com, which contains a template for a letter similar to this. The letter itself does not request any money, but the requested response to "Mr. Johnson", Claims Advisor would probably result in such request.
