I was looking for a PCR COVID test. So I started in Kenosha where I live. There used to be a container that had testing, it's not there anymore but it had the same website listed for appointments. So I looked up another one. This one in Winthrop Harbor, IL came up. On the page it has a male an appointment link...so I go there, enter my info, and asks for my DL photo.  I feel like crap so I didn't think anything about it. It also asked for my insurance card but I don't have that with me.

So I click submit it says thanks for your order. But nothing else. No appointment time, no confirmation and no email. I have more screen shots but can't upload them. So it has my DL photo.
