Recieved a request via fax for a patient-requested medication refill, to be signed and faxed back to 855-299-2482. Items requested were for diabetes supplies, pain therapy (lidocaine and diclofenac), dermatitis, fungal infections, and omega-3 fatty acid therapy. Fax was addressed to a doctor who no longer works at the office.

Company faxed paperwork over stating that a patient had requested prescription refills. The doctor the letter was addressed to is no longer working at our practice. The request was for a range of medical supplies, including a diabetes supply kit and prescriptions for pain therapy (lidocaine/EMLA cream and diclofenac), dermatitis, fungal infection, and omega-3 fatty acid therapy. The request was to be filled out and faxed back to 855-299-2482 along with the patient's medical history/records.

We received a request for medical supplies for an individual that they want the doctor to sign and fax back to them at 855-299-2482. The fax states the patient has requested refills of some medications, which she has not. Meds listed are Lidocaine ointment, or EMLA cream, and Diclofenac gel.
