I had to fill out a form that included my address to apply. I can’t remember if I included my phone number as well. The survey said it was through survey monkey. I filled out the form and got a reply a few days later saying I was chosen to evaluate a Walmart services and that I would get a survey packet in the mail. I received a check for 1895, was told to deposit the check, take 350 in payment, and buy (3) Walmart cards for 250 at 2 different stores. Then I was told to take pics of the cards and the codes on the back, and send them to the email address. I did not go through with any of this because it seemed fishy. My wife and I researched the business names and researched Walmart scams and found a similar story.
