Memphis DME Supply LLC has filed multiple claims with my insurance company. I have not paid for, purchased, ordered, or needed any medical supplies. This is a complete fraud. I had to call my insurance company at the beginning and let them know this was not a charge associated with me.

Received a claim denial letter from my health insurance company regarding Memphis DME Supply LLC billing then $4068 for 3 months of medical supplies from January - March 2026 which was never ordered or received. I have never heard of this company and have no illness or injury which would have required durable medical equipment. Luckily my insurance company denied the fraudulent claim due to lack of medical evidence and no money paid to the company. It is likely my health information was obtained by this company through a large data breach reported by ******* which occurred in October 2025 which I became aware of only a few weeks ago when MedStar informed me.

I received 2 medical claim statements from my health insurance company for 2 claims submitted for medical supplies I NEVER ordered from Memphis DME Supply, LLC located in Tennessee. Memphis DME Supply, LLC somehow obtained my health insurance card information to file false claims for supplies. I examine all of my medical claim statements carefully, and I am submitting another fraud claim with the insurance company. I also going to submit various fraud claims to put others on alert.
